About the Role:

Long Melford Medical Practice are looking for Care Co Ordinator's to strengthen their successful practice team.


The post holder will assist medical personnel in the care of practice patients, including treatment, preventative care, screening and patient education.


The post holder will work with reference to practice agreed protocols and guidelines, providing Care coordinator treatments to patients both independently and in participation with nurse colleagues and general practitioners.

Applications will be considered on a full or part-time basis hours per week.

The Long Melford Practice is a friendly, well organised and well supported PMS Dispensing Practice serving a population of 9, 500.

The Practice is located on two sites situated in the historic wool towns of Long Melford and Lavenham.

We are a well-established, well-motivated Practice committed to providing high quality health care to our local population.

As a GP training Practice we provide support to 1- 2 GP trainees and have the potential to support F1 & F2 medical students.
