Move on Support Worker

£13 - £14PAYE

Location:

Hours:
Mon - Fri, 09:00 - 17:00

Assignment: 3 months

Our social care-based client is looking for a move on support worker to work with a case load of 15 clients with no recourse to public funds, many of which do not possess immigration status, some have settled status.

A lot of the work is around making surethat all clients' health needs are met whilst they are awaiting move on and referrals to the housing team

Duties:

Accompanying clients to health appointments
Helping clients access food banks, setting up the link with food banks and then supporting clients to access the service
Helping clients access free food which is nearby
Filling in immigration forms with clients
Obtaining client signatures for consent forms for other external agencies
Being a bridge between other partner agencies so for example passing on messages about appointments
Finding out information on clients via their consent so for example obtaining information from Chain or the GPs
Being present at meetings so you can support clients to pick up on follow up actions they don't understand
Completing housing pathway referral forms for clients when they are eligible to be housed in the UK
