Description

Working hours Some opportunity to home work.

60:40 split in terms of admin/office and working in the community and networking. Possibility of occasional overnight stays required. Travel essential across Shropshire.

Hours will be flexible upon the needs of the service, providing opportunities for people at varying times responding to the needs of the users of the service (this may include evenings or weekend working)


Reports to:
Senior Community Connector and Assistant Manager.

A full UK driving licence is desirable for this role.


About the role


The 'On Your Doorstep' model of support is a new innovative model which will be adopted in Shropshire and will provide opportunities for people with learning disabilities and/or autism with mild, moderate and more complex needs to access community led support during the day as an alternative to their standard attendance at a day service using your commissioned hours.


The Community Connector in post will assist the Senior Community Connector to source activities that are specific to the people we support and meet their needs, interests and aspirations and work closely with the Senior Community Connector.


On Your Doorstep is committed to supporting people with learning disabilities and/or autism to extend their social networks, improve overall health and well-being and reduce loneliness and isolation through increased social connection.

We support development of meaningful friendships and relationships, help build confidence and self-esteem, increase independence levels and enable value in the local community.

We will offer an exciting programme of face to face social activities every 6-8 weeks based on peoples interests and wishes.


It will involve existing people from Hft's Severn Cottage service initially with the aim of attracting external people outside of the organisation to the service in the future.


The post holder will ensure that there is a positive and proactive culture in the service that ensures the people supported are able to have meaningful and productive days.

They will ensure that person centred practices take place and act as a role model in this.

The role will enable the supported people to lead fulfilling lives based upon their personal support needs.


The post holder will involve the people supported in ensuring they are satisfied with the level and quality of social activities and interaction they have the opportunity to access.
