We are able to do this by having dedicated care teams working 24/7 to ensure clients and patients needs are met but also understood from a personal perspective.
The Community Care Team Leader will ensure a quality service provision is delivered to our clients by creating a driven and motivated team of Care Coordinators who work with passion and purpose.
Your team will coordinate over 5,000 hours of care every week, so this is a vital role in improving many peoples lives.
The team act as an initial point of contact for customers and support with enquiries from clients, agency workers and the wider operations of the business.
Everyday you will
- Share the workload amongst team members and coordinate them.
- Demonstrate people management skills. Motivating and supporting the team to achieve their targets leading by example at all times.
- Act as an initial escalation point of contact for clients, families and commissioners.
- Ensure all processes and procedures are followed strictly

To thrive in this role, you must ideally have...
- Previous experience within a similar role or scope to move into this role. E.G
- Team Leader or Senior positions (within a healthcare related sector is desirable)
- Great communication skills within customer service, both written and verbal.
- The ability to work well under pressure and meet deadlines whilst remaining resilient.
- The drive to learn and grow your skillset to improve performance with yourself and your team.
