Description

The primary aim of this post is to co-ordinate and organise the care of patients with Sickle Cell Disease on an in-patient and out-patient basis.

This will include providing specialist advice, information, education and support for them and their families/care givers regarding treatments, investigations, and illness prevention to minimize acute illness and hospital admissions.

The post holder will also provide intensive support for patients admitted in conjunction with the Haematologists.

The CNS will develop a pathway for out of hours services for patients on regular transfusion regimens, will be involved in outpatient clinics and provide community support for the patients, their families, and carers.


The post holder will develop communication pathways with key stakeholders across health and social care settings and with their community nursing colleagues and Guys & St Thomas Clinical Haematology MDT.

The CNS will be involved in management tasks such as the development of policies, quality, and audit initiatives and service improvements.
