Hospital Discharge Social Worker


A Local Authority based in the Merseyside region is seeking an experienced Social Worker to join their Hospital Discharge service which operates across 2 hospitals in the area.


Highlights:


  • Will have the option of working across 2 hospital in the area
  • The role will focus on safely discharging patients back into the community or intermediate care
  • Although this is a hospital discharge team you will have the opportunity to work from home providing a higher degree of flexibility.
  • Pay rates for this post are up to £35/Hr which is above market rates in the region giving you the opportunity o boost your earning and have greater disposable income
  • As you will be hospital based you wont have to spend time travelling to visits saving you money and time.
  • You will be joining a largely stable team with supportive managers that are accessible to provide guidance and sign of on decisions


This is an excellent opportunity to take on a hospital post which offers a a higher degree of flexibility as you will have the opportunity to work from home saving you some time and money on commuting.

The post will give you the option of working across2 Hospitals in the area to safely discharge patients back into the community or intermediate care services safely


The team is a largely stable team and information is shared between services effectively so as a Social Worker you will have all the information required to be able to make effective decisions.

Furthermore you will be supported by a long standing managementteam who are always available to help support with your decisions so that you can be confident in the care plans that you propose.


To be considered for this post you should have experience of working with Adults in the capacity of a Social Worker.
