The purpose of this post is to work as part of the Enhanced Support time and Recovery Team, and the Rehabilitation and recovery service, which provide mental health services to those individuals who have complex needs, and to focus on the direct needs of the service users


They will:

  • Provide support and give time to an allocated group of service users, and promote their recovery supporting their return to a community environment, or supporting them to maintain their place in the community
  • Support service users to meet their identified needs and outcomes, paying attention to and respect their values, wishes and aspirations to lead their lives in the way they choose, supporting them to access community resources and services, such as housing, benefits, leisure, education and employment
  • Assist the Rehabilitation and recovery service and the ESTR team to assess, plan, implement and evaluate mutually agreed goals and recovery plans.
  • Work with the service users to further develop personalised and detailed outcomes and goals.
  • Be able to work independently with service users, responding safely and flexibly to changing needs.


This is an exciting opportunity for an experienced Support time Recovery worker wishing to join the community mental health primary care team in Conwy.
