Job summary
Eastleigh Health Primary Care Network (PCN) is looking for two Mental Health Care Coordinators to join our growing team.


The successful applicants will work within the PCN Mental Health Team, providing extra time, capacity and expertise to support patients with their mental health needs.

The role involves providing a first point of contact for our patients with mental health needs, providing coordination and navigation for patients and their carers, ensuring they are referred to the correct service, for example a Health and Wellbeing Coach, a Social Prescriber or even an external service.


We are looking for compassionate and motivated individuals with strong administrative skills, and the ability to communicate effectively with patients and wider external agencies.

You will become the first point of contact for some of our most vulnerable patients. The ability to listen and communicate clearly and effectively is therefore vital to this role.


You will be joining a growing team of Mental Health Workers, Social Prescribers and Health and Wellbeing Coaches, and will be an integral part of the team when it comes to delivering the best patient care.


Main duties of the job
As a Mental Health Care Coordinator you will work as a key part of the PCN multi-disciplinary team (MDT). You will be the first point of contact for many of our most vulnerable patients.

Care Coordinators provide extra time, capacity and expertise to support patients in preparing for or in following-up clinical conversations they have with primary care professionals.


You will work closely with the Primary Care Mental Health Team to support a case load of patients, making sure that appropriate support is made available to them, and their carers.


About us


Eastleigh Health PCN is comprised of the four practices in Eastleigh - Archers Practice, Boyatt Wood Surgery, Parkside Practice and St Andrew's Surgery.


With a patient population of around 33,000, these practices are working across Eastleigh to deliver high-quality primary health care for our patients.


St Andrew's is the lead practice for Eastleigh Health, and will be the employer of the Mental Health Care Coordinators, but you will work across all four practices.

Key Responsibilities

  • Refer patients to other health professionals within the PCN, using their experience to screen patients, with the aim of identifying which service the patient would benefit from.
  • Support external referrals where appropriate.
  • Liaise and build effective working relationships with external agencies.
  • Provide administrative support to the PCN Mental Health Team as appropriate.
  • Support patients to use decision aids in preparation for shared decisionmaking conversation.
  • Communicate effectively at all levels of the organisation, to a variety of health professionals, carers and external stakeholders to provide the best patient outcomes.
  • Help people to manage their needs through answering queries, making and managing appointments, and ensuring that people have good quality written or verbal information to help them make choices about their care.
  • Support people to understand their level of knowledge, skills and confidence (their 'Activation' level) when engaging with their health and wellbeing.
  • Assist people to access selfmanagement education courses, peer support or interventions that support them in their health and wellbeing and increase their activation level.
  • Explore and assist people to access personal health budgets where appropriate.
  • Provide coordination and navigation for people and their carers across health and care services, working closely with Social Prescribing Link Workers, Health and Wellbeing coaches, and other primary care professionals.
  • Produce accurate, contemporaneous and complete records of patient consultation, consistent with policies and procedures.
  • Raise awareness of how to identify patients who may benefit from shared decision making and support PCN staff and patients to be more prepared to have shared decisionmaking conversations.
  • Support the recall of patients with longterm conditions who require appointments.
  • Support and raise awareness of national screening and immunisation programmes and other health promotions, encouraging participation to ensure targets are achieved at a practice and PCN level.
  • The post holder would be expected to undertake a suitable course accredited by the Personalised Care Institute.
  • The post holder will need to have access to own transport and theh ability to travel across the locality on a regular basis.
