The priory group are looking to recruit dedicated support workers to work on a outreach basis in the Leeds area to support a gentleman to access the community within the hours Monday to Saturday 11am till 5pm and Sunday 9.30am till 3.30pm.

The Priory Group provides assessment and support for adults living with mental health diagnosis, and complex needs. Our outreach services offer 24/7 specialist support in the community with in the service users own home.

This model of care is designed to provide just the right level of support; we encourage individuals to develop the skills that they need to live as independently as possible.


As a support worker with in this home you will be required to support the individual to access community involvement and professional appointments, be proactive around meaningful activity, provide emotional support and also have an understanding around Pradwe-willi syndrome however full person centred training around this individual will be provided.

As a support worker you must be committed to working in a person centred way that promotes independence.

You will be required to be flexible within your working pattern.


You will be expected to provide support in a way that reflects the wishes and needs of the individual you are supporting, and is consistent with their individual support plans.

You will be expected to comply with all regulation as defined in policies, in supervisions and in training sessions.


You will also be expected to attend supervisions, team meetings and training and ensure that you provide good communication to your manager and colleagues.

You should have initiative and motivation to work alone, as well as part of an team.

You must be able to relate to people; be a good listener and communicator.


It is not necessary to have experience of working with people with prader willi-syndrome although this would be an advantage.


However, you must demonstrate an understanding of the issues faced by people with mental health and an enthusiasm for working and supporting them to take control of their lives.

A good basic level of education is desired.
