My client, a Southeast London local authority, are recruiting Case Management Officers to support the flow of psychiatric patients from hospital settings to the community.
These are temporary positions until 31/03/22 in order to manage additional demand on services over the winter period.

The roles are fulltime 35 hours per week Monday to Friday, core hours are 9am to 5pm.
Based onsite at the Ladywell Unit, University Hospital Lewisham the Case Management Officers will work closely with the SLaM Flow Team, Hospital Social Works Team and Community Mental Health Teams.
CMOs will attend daily flow meetings and screen new inpatients to identify and prioritise patients with potential barriers to discharge at the earliest possible stage.
CMOs will also support the discharge arrangements for Lewisham patient placed in outofborough hospitals and general hospitals.
In conjunction with MDT colleagues CMOs will put in place action plans to address practical barriers, with a particular focus on practical and accommodationrelated issues.
CMOS will meet with inpatients and carers on the wards and will also carry out home visits and other appointments in the community to support discharge (e.g. accompanying patients to housing and benefits appointments) and may involve followup of practicalissues following discharge.
