Description

The focus of this role is to lead, support and facilitate the development of integrated Neighbourhood SMI and SMI with Frailty pathways.

Working in partnership with the wider NEE Alliance health and social care economy including third sector organisations. Working to minimize inappropriate admissions to both general hospital and mental health inpatient settings.

You will be expected to work collaboratively with service users, carers, clinicians and other practitioners within the multi-disciplinary team in delivering services You will have continuing responsibility for proactive assessment, monitoring and management of patients and ensuring care is current, evidence based and in line with practice standards.


You will provide clinical leadership to the team in evaluation and monitoring of the service, provide specialist assessment as per the service users, and carers requirement You will provide management/clinical leadership to the team in evaluation and monitoring of the service In delivering your duties you are expected to display behaviors that are aligned with the Trust values and as required in accordance with the NHS Constitution at all times.
