SALARY:
£21,222 per annum


JOB PURPOSE AND ROLE:

To ensure all referrals and direct contact from clients into dementia services are actioned in a timely manner.

Taking appropriate steps to signpost, provide information or arrange for onward referral to relevant internal or external services quickly and efficiently.


To be responsible for delivering excellent client care and accurate recording of actions and outcomes in accordance with the Age UK Herefordshire & Worcestershire procedures and operational standards.


To ensure that all enquiries are answered and dealt with efficiently and accurately ensuring the number of missed calls / unanswered queries is kept to a minimum.


An Enhanced DBS Check will be required

PRINCIPAL RESPONSIBILITIES AND DUTIES:


  • To process initial enquiries, within agreed operating practices ensuring accurate and timely records are kept on Charitylog.
  • To ensure any customer feedback is recorded promptly and accurately and escalated if appropriate.
  • To maintain the dementia information resources
  • To adhere to the principles of good customer care, obtain and analyse regular feedback from users.
  • To keep informed of developments in local services, including attendance at training and updating events.
  • To maintain working relationships with officers from both statutory and voluntary agencies to support effective signposting and referrals.
  • To maintain records in accordance with quality standards and the Data Protection Act.
Input client and project information onto Charity Log.

  • To undertake project work for post diagnostic dementia services.
  • To undertake administrative tasks in support of the Service including statistical analysis and report writing with appropriate training.
  • To be willing to travel and work flexibly throughout the charities operational area
  • To be prepared to work from any of our operational bases as required
  • To work in accordance with GDPR, Safeguarding, best practice and quality frameworks.
  • To take responsibility for own personal development and learning. Undertake training as required.
  • To positively promote and publicise the work of Age UK H&W.

GENERAL

  • To undertake other such responsibilities and duties as may be required by your line manager within the level and grading of the post, working flexibly as required.
  • To work collaboratively with others in achieving the aims and objectives of Age UK Herefordshire & Worcestershire set out in the Organisational Strategic Plan.
  • To work within the policies Procedures and quality frameworks adopted by Age UK Herefordshire and Worcestershire with particular attention to equal opportunities, data protection/confidentiality, safeguarding and health and safety requirements.
  • To participate in relevant audits and continuous improvement programmes, to ensure service delivery is maintained in line with internal and external standards.
  • To participate in the supervision and appraisal systems adopted by the Organisation.
  • To undertake relevant training as required by the organisation and agreed with your line manager.

  • To undertake such other duties as may be reasonably required, consistent with the nature and grade of the post.
