Description
The post holder will work as a member of the multi-disciplinary community rehabilitation team as a dual diagnosis specialist. They will offer assessment, consultation and support for patients under the care of the team.

The post holder will work in close collaboration with patients, families and other professionals and services involved the persons care, including acute trust colleagues, mental health trust teams, primary care, social services, housing and homelessness services, substance use services, schools and colleges, ambulance services, the police and other criminal justice agencies.

The post holder will use an integrated motivational interviewing approach that is person-centred and holistic which considers the needs of others in the service User social system.


They will be required to conduct a comprehensive biopsychosocial assessment which considers the relationship between the persons mental and physical health, their social situation and their harmful use of drugs and alcohol in formulating person-centred and needs-based care plans.

The post-holder will be required to assess clinical risk and to prepare and implement risk minimisation plans that, in consultation with senior colleagues, deliver effective safeguarding for the individual and others around them.

The post-holder will act as a conduit to engaging service users in community based drug and alcohol services and offer outpatient clinics and follow-up.

They will participate in clinical reviews of patients, working with individuals to develop bio-psycho-social care plans.


The post-holder will lead on, and work with the supported housing teams, to promote a safe environment for service users and staff whilst maximising recovery values.

The post-holder will work with colleagues from all disciplines to develop their knowledge, skills and confidence in working with service users with co-existing substance use difficulties so that it is a core aspect of the work for all team members.
