Job Title:
Haringey Homeless Health Inclusion Team (HHHIT) Care Navigator


Role Summary:


The Care Navigator will support patients experiencing homelessness to engage with health and social care services and to reconnect them with primary care.

Patients in our service have multiple needs crossing various agencies:
drugs and alcohol, mental health, social services, housing support, and primary care.

Therefore, the role involves working across several sectors to help patients navigate the complex health and social care systems, overcoming access barriers.


You will work within a multi-disciplinary team and allied agencies to provide individual holistic assessments, while signposting and referring patients to appropriate services and activities that may help to promote their health, wellbeing, and independence.


Therefore, it is imperative that trauma-informed care is provided, which includes understanding, recognising, and responding to the effects of all types of traumas.

Effective care navigation is crucial to delivering coordinated, patient-centred care and supporting patients in accessing healthcare appropriately.

You will be working to deliver the key objectives of the service: supporting and strengthening access to General Practice for patients experiencing homelessness in
Haringey.

Main Duties and Responsibilities (Administrator):


Overview

  • Provide support for the development and delivery of the Federation's Haringey
Homeless Health Inclusion Team as a service: o GP Registration o Patient engagement o MDT Meetings o Outreach Service o Educational Activities of the service o Monitoring and Evaluation of the service o Referring/signposting to appropriate services o Pro-actively monitor caseload to identify outstanding Key Performance

Indicators (KPIs) to facilitate timely discharge of patients.


1. Administration Responsibilities

  • To keep accurate and uptodate records of contacts with patients (including the use of EMIS)
  • To provide administrative support for meetings within the service, including the preparation of meeting agendas, room booking, calling for papers, minute taking, action tracking, and updating risk registers.
  • To be responsible for maintaining systems of storage and the retrieval of information, ensuring that information is accessible to other members of the team.
  • To ensure that the day to day running of the HHHIT service is smooth, including the maintenance and ordering of equipment and supplies.
  • To maintain and update the GP registration database to an accurate level.
  • To manage the patient list for the HHHIT MDT meeting, accepting, and removing patients from the register.
  • To assist the Lead Outreach Nurse on their outreach clinics and to follow up with patients where necessary.
  • To maintain and update the referrals tracker workbook.

Main Duties and Responsibilities (Care Navigator):

  • To register patients to GP Practices, reengaging them with primary care and what it offers.
  • To minute the MDT and associated meetings, while communicating a patient's needs and surrounding circumstances.
  • To perform holistic health checks alongside the Lead Outreach Nurse on outreach clinics, identifying the patient's wider needs and supporting them with accessing the right services.
  • To actively research appropriate services to develop a database/network for health, social and voluntary sect
