About the Team

The Hospital Adult Social Care Team is a vibrant and fast paced team, providing social care support to all North Northants residents who need an assisted discharge from hospital.

We take referrals for North Northamptonshire residents from all hospitals, but predominantly from Kettering General Hospital (KGH) Northampton General Hospital (NGH) and the Medical Rehabilitation hospitals run by Northamptonshire Healthcare Foundation Trust (NHFT).

In line with current national guidance, we operate a Discharge To Assess (DTA) process which requires us to carry out timely assessments for those people discharged to DTA beds in care homes in the North of the County and for people discharged to their own homes with DTA packages of care or to the two Specialist Care Centres (SCC's) in the North of the County.

The position is based at Kettering General Hospital (although the expectation would be that you are flexible across all the sites we cover in North Northamptonshire if required)

What will you be doing?

The main duties of the role are supporting the Team and SBSO to keep crucial spreadsheets and data bases accurate and up to date (this function is crucial to the smooth running of the Team and supporting with timely discharges).

Additionally, you'll be answering the phone along with other administrative tasks to help with the running of the busy Hospital Social Care Team.

Support will be given on all aspects of the role from both team members and the SBSO within the Team.

About you
Applicants will be able to demonstrate good communication skills and be confident on the telephone.

You will ideally have a good level of IT skills with a sound knowledge of Word, Outlook, Teams, and Excel and have a good attention to detail.
