The Community Matron team provide a valuable service working with patients to help them to maintain independence and well-being in their own homes and communities, avoiding admission to the Acute Hospital.

The team provides Medical, Physiotherapy, Occupational Therapy, Social Care and Nursing support and intervention to older patients with complex needs.

We give patients time and attention working to consider all aspects of their mental, physical and social health.

The care coordinators are fundamental in supporting the clinical staff with this aim, and are valued staff members. We look forward to welcoming you to our friendly team. You must have audio typing skills, and have had previous experience.

If you are efficient in prioritising your own workload and enjoy patient contact, and liaising with outside agencies, this role could be for you.

Other tasks will include document management and dealing with incoming post and queries.

You will also have a responsibility for supporting the Community Matrons with the organisation of their clinical and patient related activities.


  • Opening and distributing all incoming post
  • Calling to arrange meetings
  • Exchanging verbal and written information with patients, staff and carers relating to appointments
  • Provide high quality comprehensive administrative service
  • Uploading documents
  • Typing of correspondence Cluster meetings
  • Data Input
  • Cluster work as wider MDT

The 4"must haves" for you to be considered for this role:

  • Car driver with ability and readiness to travel to multiple sites
  • Able to demonstrate awareness of own learning needs
  • Ability to manage deadlines, plan and organise own workload
  • Previous experiences relevant to post
