Description

Job Purpose To work as part of the Integrated Respiratory Service, providing integrated health and social care support to patients and carers in both clinic and exercise group as well as in the home environment.

To undertake technical, clinical and a variety of other appropriate activities following training and demonstration of competency, delegated by the relevant profession To work without direct supervision undertaking delegated tasks in order to support the team To report back to the team anything that may effect or contribute to the future care of the patients or carers Key Responsibilities Assist in delivery of Pulmonary Rehabilitation sessions.

Assist in managing patient caseloads. Addressing Health and social need.


Implement programs of care assessed by Health and Social Care Professionals to address peoples health and social care needs in a variety of settings Contribute to the reduction of health inequalities through the delivery of agreed health promotion interventions e.g.

Smoking cessation Undertake regular telephone duties and be able to advise patients appropriately and escalate to a trained member of staff any concerns.

PATIENT AND PUBLIC INVOLVEMENT Actively encourage comments and feedback about the services provided QUALITY Ensure that own actions help to maintain the quality of the service.

Maintain a professional image To recognise and respond to the needs and choices of the individual when delivering care RECORD KEEPING Ensure that own record keeping meets local standards COMMUNICATION Establish and maintain communication with people about routine and daily activities, modifying communication to overcome any difficulties with people involved Provide the team with accurate and timely feedback on interventions PROFESSIONAL DEVELOPMENT Contribute to own personal development and identifies own learning needs through active participation in the performance review and development process and maintaining own portfolio Participate in the supervision framework as a supervisee TEAMWORKING Support effective team working within the team Acts as an effective team player SERVICE DEVELOPMENT Assist in the development, ongoing review and marketing of the service Assist in data collection for research and audit purposes Awareness, understanding and a commitment to the pursuit of equality of opportunity in terms of service delivery and employment practice HEALTH SAFETY AND SECURITY Assist in measures which maintain standards that ensure the health safety and security of self, patients / clients and colleagues Ensure that confidentiality is protected at all times Practice in accordance with policies and guidelines for the Protection of Vulnerable Adults and Child Protection
