Description

This is a new and exciting post to support the development and delivery of the mental health care offer within Primary Care as part of our transformation of mental health services in Dorset.


The post holder will be based and embedded within the Primary Care Networks in Poole (PCN) and supported by the Poole Community Mental Health Team.

The role will entail line management of the Specialist Mental Health Practitioners based within Primary Care (ARRS Roles) as well as the Physical Health Support Workers.


The role will require working with GPs, practice nurses and other Additional Roles Reimbursement Scheme (ARRS) workers e.g., Mental health practitioner, social prescribers, health and wellbeing coaches, care co-ordinators, as part of the wider primary care Multidisciplinary Team (MDT), and will also involve many other valuable services from the voluntary and community sector.

You will work with primary care GPs to manage the mental health needs of their population at the point of contact.

This includes those with lower-level support requirements that can be met within primary care.


The role will be key in developing the relationships and interface between Primary Care and Secondary Care Mental Health across the Locality as well as supporting the evolving transformation of Mental Health Services in Dorset.
