An opportunity has arisen within the trust, for a fixed term period of 12 months, to take up the opportunity of Healthcare Navigator.

This role is fully explained with in the job description.

The role will be embedded within the Experience Team and will be evaluated during the year period.

This is included in Job description link working with Patients in Northumberland area

  • To be able to collect and input patient information onto the Healthcare Navigator databases and analyse a range of data to identify where changes have been made to patients and their lifestyles.
  • Work sensitively with patients, their families, and carers to capture key information, enabling tracking of the impact of the Healthcare Navigator role.
  • Encourage people, their families, and carers to provide feedback and to share their stories about the impact of the Healthcare Navigator role the patient journey.
  • Use the case management systems to track patients progress on a clinical pathway.
  • Provide appropriate feedback and datasets to groups and forums to update on the progress and performance of the Healthcare Navigator pilot.
  • Support evaluation of the pilot.
  • To develop rapport with patients to identify need and facilitate support
  • To support patients to find and access the right information and appropriate signposting to services.
  • To identify and manage any risks relating to patient contact, particularly when engaging with patients alone.
  • Resolution of patient presenting issues and proactive management and escalation of any risks.
  • Provide personalised support and advice meet people on a onetoone by phone, video conference, or facetoface. Give people time to tell their stories and focus on 'what matters to the person'
  • Build trust and respect with the person, providing nonjudgemental and nondiscriminatory support, respecting diversity and lifestyle choices.
  • Work from a strengthbased approach focusing on a person's assets.
  • Establish and maintain links with relevant internal and external posts, teams and peer networks who focus on similar outcomes for patients.
