About the team:
Lambeth Single Point of Access (SPA) are a team of mental health professionals, support workers and social workers who can help you access; one of three Living Well Centres.
These offer short and longer term support from our teams of clinicians, social workers, people with lived experience and support workers; or if appropriate, whether a team that can visit you if you require mental health crisis support within 24 hours; as well as other services and community groups who can support you.
We are a part of the Lambeth Alliance, and we have a close interface with a variety of SLaM services, other statutory organisations (inc. social care) and voluntary sector.
The Lambeth Living Well Network (LWN) Alliance brings together Certitude, Lambeth Clinical Commissioning Group, Lambeth Council, South London & Maudsley NHS Foundation Trust and Thames Reach organisations to deliver an innovative way of providing mental health support for those who need it in the Lambeth Borough; a fully integrated and coordinated mental health system which focuses on people's strengths and supports their whole health and wellbeing.
As part of our journey, to realising a fully integrated system, we have set up 3 Living Well Centres in Lambeth.
About the role:
You will be expected to lead and manage the team, work in partnership with the clinical expert and other senior members of the team to provide management responsibility for the delivery of care within an integrated community team, in accordance with trust and Alliance policies and procedures.
You will co-ordinate multidisciplinary working, maintaining excellent communication and working relationships with all team members, and other services within the Trust and the Alliance and to ensure effective interface and continuity of care between disciplines and services.
The post holder will take a lead in the development of identified areas of professional practice within the service to ensure high levels of clinical excellence and that training and professional development needs are met within the team.
Take responsibility for the provision of regular supervision and appraisal, of identified team members, ensuring the delivery of high-quality evidence-based practice, within the overall structure of the team.
Working as past of the MDT you will ensure a timely, safe, effective and responsive service delivery in line with the LWN Alliance "three big outcomes" for people living in Lambeth who experience mental distress:
