Are you an experienced Nursing Assistant that is passionate about keeping patients in the comfort of their own home to avoid hospital admissions? Then this is the role for you.
As part of the Discharge to Assess Team you will be part of a wider multidisciplinary team which includes Occupational Therapists, Physiotherapists, Nurses and Pharmacy.
We will support you with specialist and bespoke training to reach your potential and excel in your chosen career path.
The Discharge to Assess service will be a seven day service working between the hours of 8am-9.30 pm on a rota basis.
In the role as Senior Nursing Assistant, working without direct supervision you will be responsible for delivering clinical care for patients in their own home.
You will be responsible for your prioritising your own caseload of work and be responsible for tasks delegated from a registered nurse.
Communicating effectively with patients, relatives, carers, and other healthcare practitioners, ensuring any concerns are listened to and relevant actions are taken.
A full UK driving licence with access to a vehicle is required for this role as you will be Travelling across the Trafford Community locality.
