Description

We have a vacancy within the Integrated Discharge Team for an enthusiastic Band 4 with an eye for detail who enjoys a challenge.

This is currently a six day service with the planning in place to move to a seven day service.

We will offer in house training and full support.

You will be an active member of the Integrated Discharge Team, assisting with co-ordinating and liaising with services to manage complex discharges, encouraging multi-disciplinary working and communication.
