It is an innovative new role which will be key to developing the Health Visiting and School Nursing contribution to Family Hub agenda forward.

As a senior practitioner you will be a key part of the service leadership team and lead in a manner that role models the Trust values.


An exciting opportunity has arisen for a dynamic, motivated and compassionate qualified Health Visitor or School Nurse with substantial experience working as a SCPHN in the HCP 0-19 programme.

You will need to

  • Demonstrate your ability to lead teams through times of change,
  • Have an excellent record of working in partnership with other agencies and services
  • Have experience of engaging staff from within the service and across our partnership in health, education and children's services.
  • Work with children & families to coproduce the provision.
Have experience of using best evidence and practice to develop and deliver pathways of care within a strong governance framework

This is an important role to help achieve the Walsall Together agenda.


To lead the Healthy Children Programme 0-19 (Health Visiting and School Nursing) contribution to the Early Help and Supporting Families partnership agenda.

This will be established through collaborative working with other health, social care, education providers, primary care and voluntary sector.


The role will lead in one locality of the borough, aligned to a locality family hub and be responsible for shaping the delivery of the HCP within the locality.

To identify and co-ordinate the delivery of the HCP offer working with the Clinical Team and professional leads.


The post will support the access for children, young people and families to the most appropriate health provision so that they access the 'right help at the right time'.

This will be established through collaborative working with other health, social care, and education providers.


To hold a caseload of children, young people and families with complex needs and take on lead professional role for Early Help level 3.


Walsall Healthcare NHS Trust is an integrated Trust and the only provider of NHS acute care in Walsall, serving a population of 286,700, providing inpatient and outpatient services at the Manor Hospital as well as a wide range of services in the community.

Walsall Manor Hospital houses the full range of district general hospital services under one roof.

The £170 million development of our Pleck Road site was completed in 2010 and the continued upgrading of existing areas ensures the Trust now has a state-of-the-art Critical Care Unit, Neonatal Unit, Obstetric Theatre, and Integrated Assessment Unit facilities.

Work is also well underway on its multi-million pound new Urgent and Emergency Care Centre which will house a new Emergency Department (including Children's Emergency Department), co-located Paediatric Assessment Unit, Acute Medical Unit and Urgent Care Centre.

This is scheduled for completion this year.


We provide high quality, friendly and effective community health services from some 60 sites including Health Centres and GP surgeries.

Covering Walsall and beyond, our multidisciplinary services include rapid response in the community and home-based care, so that those with long-term conditions and the frail and elderly, can remain in their own homes to be cared for wherever appropriate.

Partnership Working and Service Development & Leadership

  • Takes lead in the development of positive relationships across the Early Help partnership including engagement of staff within the HCP 019 service
  • Takes the lead on the development of the HCP 019 Core offer to the Early Help agenda working with the deputy and professional leads
  • In collaboration with key partners promote a working environment, which supports change, innovative practice, and new ways of working to support service development, address inequalities, diversity and improve services for local people.
  • In collaboration with key partners contribute to the development of strategies/workstreams to meet the needs of service users
  • Support development of strategies to engage with children, young people and families/carers to ensure their involvement in their care.
Clinical Responsibility for delivery of care

  • Takes a lead in specialist area/complex caseload and provides advice and support to others, including taking on Early Help lead professional role or supervising junior staff in lead professional role for less complex cases.
  • To be responsible and accountable for the assessment of care needs, the development, implementation, and evaluation of care, ensuring that children, young people and families receive the highest quality of care.
  • Demonstrate analytic and a high level of judgemental skills when interpreting and performing
