Are you a Mental Health Practitioner looking to work as part of a collaborative, developing community team based on the outskirts of Norwich?


Hello, My name is Nicole Smith and I am the Service Manager here at the City Community Mental Health teams based at The Anchorage, Norwich.


Our forward-thinking community mental health teams are seeking a mental health practitioner to join their team as a dedicated caseload holder.


We are currently improving on how we work, to ensure that our team can provide an excellent service to our local communities; we have recently invested in numerous clinical lead posts to support our current multi-disciplinary teams to ensure we have nurse consultants, clinical nurse specialists, psychologists to name a few.


As a caseload holder, you will hold a caseload of service users with complex mental health conditions (such as bi-polar disorder, personality disorder, schizophrenia, severe depression) that require ongoing care and treatment from secondary mental health services.


It is the ideal time to join our collaborative team as we transform and develop the way we work -although this is a full-time role, we can be flexible and can offer some hybrid working and/or flexible working patterns if needed.

You'll play a key role in our service users' recovery; you'll complete a recovery care plan collaboratively with them and review this regularly with the aim of the service user being discharged from the service as part of their recovery journey.

You will work with the service user, carers, external agencies and other services to ensure the service received is streamlined and cohesive.


You'll be working alongside a multi-disciplinary team and will be required to undertake assessments and the role of the duty worker.

You'll regularly be working with external agencies as part of a service users care and treatment, so being able to work well in a team and having excellent communication skills is vital.

You'll get the opportunity to work with students from various disciplines as well as supervising other members of the team.

You can expect a thorough induction and training period, as well as opportunities for ongoing development.

You can expect a manageable case load and a supportive management team to ensure you're able to work independently but support is never far away if needed.
