The role will cover the following areas' Sedgley, Coseley and Gornal

Are you the right person for the job?

Experience in supporting vulnerable adults in a personcentred way
Knowledge of strengthbased recovery models and approaches

What will your role look like?

Provide holistic onetoone social prescribing support for people aged 18 and over who are frequent presenters at GP surgeries
Entail actively contributing as a member of a wellestablished social prescribing team who supports the most vulnerable in society

Mileage is paid for all posts at NJC rates where applicable
