Kingfisher Mother and baby unit is excited to recruit a Band 4 Assistant Practitioner discharge co-ordinator to join the team.


As an Discharge Co-ordinator you will have the opportunity to use and develop your knowledge and skills whilst supporting the patient through their admission journey, ensuring the discharge process is a smooth as possible to reduce risk of readmission.

There will be good opportunity for further development around medication management and physical health.

We are a forward thinking and flexible team and we are looking to do things differently.


Kingfisher Mother and baby Unit is an admission ward with the role to assess, treat and identify future care needs for the patients.

The ward has 8 bedrooms providing care for up to 8 mothers and their babies.

The role would entail supporting the team in collaboratively assessing, planning, delivering and evaluating care for those we are privileged to look after.

The post holder will be responsible for the effective coordination of patient discharges from the Mother and baby Unit and will work closely with the outreach team, hospital clinicians, social work teams, personal care services and multi-disciplinary team members to deliver a high quality, patient focussed service.
