The Children Looked After Health Team in Islington is seeking a Patient Pathway Coordinator.

This is a fixed term contract of 23 months in the community to cover a secondment with the possibility of an extension to a permanent contract.


The post holder will proactively manage and track the progress of all notifications to ensure each stage of the pathway runs smoothly, whilst also liaising with colleagues in and out of borough.

You will be required to assist in data quality and have a high level of competence in IT skills. Previous administrative experience in the field is desirable and the ability to work in a constantly changing environment. You should be able to manage your time effectively and use your own initiative. You will receive regular supervision from the administrative team lead and will be supported in your professional development.

Produce a variety of documents from Dictate IT and other formats.

Produce documents quickly and accurately to a tight deadline

Produce reports, statistical information and analysis using various IT packages and data bases.

Support the service with on going audits. Complete monthly/quarterly data sheets.

Undertake logging and distribution of incoming mail. Photocopying, processing of correspondence, filing, scanning and uploading and general clerical duties for the team.

Use a range of communication skills and initiative appropriately.

Advise and deal with queries, requests from a wide range of internal and external agencies.

To arrange, plan and organise meetings. To prepare for weekly team meetings and follow through actions.

To maintain health assessment clinic and bookings. To circulate health assessments reports. To liaise with social workers, young people, residential placements and carers to support this process.

Ensure confidentiality is maintained and vulnerable children's information is safeguarded. To communicate sensitively with clients.


To assist in the induction of temporary and new permanent members of Staff and act as a support re IT issues where appropriate.

To manage Adult Health records.

Process team oyster cards.

To obtain laboratory results from Pathology Department.

To carry out any other duties commensurate with the grade as directed by the line manager.
