Community Outreach Worker - Role


This is a statutory role whose primary purpose is to support the housing improvement project(SHIP) whilst working alongside Plymouth Adult Safeguarding team.

This role also encompasses other statutory duties and responsibilities of Adult Social Care as defined by legislation and guidance.

There has been a rapid rise in new providers delivering supported housing using the exempt model in Plymouth.

Large landlords from other areas have been setting up supported housing for vulnerable client groups, including the homeless, prison leavers, individuals with mental health challenges, substance misuse, learning difficulties and physical disabilities using the exempt model.

As a consequence, there has been an increase in anti - social behaviour and less targeted support to individuals, the role will require the monitoring of the supported housing services, in terms of the housing or support or contact with residents.


The successful applicant will work with the Supported Accommodation Review Team (SART) to look at single individuals living in the accommodation who often have multiple needs.

As a consequence of these needs the Local Authority are evidencing increasing significant impacts for individuals and communities.

The position offers scope to effect high quality social workpractice with an emphasis on collaborative working, social justice, and creative interventions for people who could, for a multitude of reasons, be at risk of harm.

Overall focus will be to play acentral role in developing and improving outcomes for people whilst reducing risk.

Experience of working with people experiencing multiple disadvantage would be beneficial
