We are pleased to offer a vacancy for a Band 5 RMN 37.5 hours within our community memory assessment service in Bolton.
The applicant should be highly motivated, innovative, and enthusiastic with a passion for person centred care in dementia and older adult nursing.
You should have an interest in advocating the needs of older adults and will have the responsibility of a defined caseload to monitor medication titrations and support post diagnostic interventions.
You will provide care and interventions using evidence based practice and client centred principles to achieve positive outcomes and will liaise with community partners to ensure continuity in care.
Be responsible for a small caseload of patients as lead professional.
To oversee medication titrations and medication monitoring
To liaise with GP regarding medication changes and ensure medications are supplied.
To fully participate in the continuous improvement of the service and the implementation of the operational and clinical policies.
Promote opportunities for local service development.
Professional Conduct, Professional Standards and National, Trust and local Policies and Procedures.
Demonstrate the sharing of knowledge, skills and attitudes to others.
Participate in multi-disciplinary meetings with the aim of improving clinical practice.
Must be willing to travel across the Bolton borough to undertake home visits, this will require the use of a car
- travel expenses will be paid.
Must demonstrate an understanding of our trust values.
