Job overview:
Working with multiple teams and organisations across Cornwall the post holder will support End of Life care to develop system wide co-design programmes with key stakeholders and support the definition of the strategic direction of integrated End of Life models of care in Cornwall.
Seeking, sharing and utilising information with other multi professionals including cross boundary working to deliver and sustain high quality evidenced based care and identify skills and services to achieve end of life care closer to home.
Interview date - 15th June 2023 to be held at Venton Conference Centre, Chapel Parc, Newquay, TR8 5AHMain duties of the job:
Work alongside team managers to support staff in developing knowledge and skills to provide high standards of end of life care.
Support clinical advice to serious incident investigations and clinical reviews when end of life care is the focus of the incident.
