An exciting opportunity has arisen to join the Trust's Referral to Treatment (RTT) Team as a Pathway Coordinator.
The RTT Team leads at the hospital on the data assembly, validation and the redesign work required to achieve the national target and to ensure patients are seen and treated in a timely manner.
To undertake this role, you will need to have a positive and pro-active approach to work, have the ability to prioritise and be flexible in your approach when necessary.
You will also need to have a strong team working ethic, able to work on your own initiative and have excellent communication skills.

To validate RTT pathways, ensuring errors are identified and rectified
By using appropriate Trust systems to capture and record accurate data
To provide general administrative support to the RTT team

Please review all documents attached to this advert to ensure you familiarise yourself with all requirements of the job.Ensure patient referral and outcome information held on the Trust's PAS system accurately reflects patients' RTT times
To review patient records and use information within to validate RTT data
To promptly report any identified data quality issues or potential wait time breaches to the RTT Manager
To liaise with relevant clinical teams to ensure quality of validation is consistent with clinical practice
To work closely with the Information Department to understand data quality issues and the impact on RTT reporting
Use PAS, Evolve, ICE to formulate pathway data on individual patients
To unravel patient pathways in an attempt to understand the various stages of the patients care and pathway
To source, collect and validate accurate RTT data in accordance with agreed practices and standards
To support the monitoring of the RTT target
To participate in planned training events and to attend all mandatory training sessions as requested
To record accurately the reasons for patient breaches
To collect additional data on an adhoc basis as required
To plan and organise own work schedule with the use of appropriate planning aids, demonstrating good time management and ensuring anticipated difficulties in meeting deadlines are promptly reported
Guide services in regular routine validation of waiting lists to ensure the waiting lists are accurate at all times
Respond to requesting Trust IPT requests
To provide the first point of contact for written and telephone enquiries on behalf of the RTT Team using own initiative & judgement to establish the validity and priority of the contact
Assist with audits
To undertake any other duty as requested by your manager which is essential to the smooth running of the RTT Team
