Description
The role of the Assistant Practitioner is to deliver high quality, caring and compassionate patient care.

The post holder will undertake assessment, implementation, and evaluation of patient care needs for a defined group of patients under the indirect supervision of a registered professional in their specialist area.

The post also includes facilitation of telehealth monitoring in the community for a group of patients.

The post holder is required to work as part of a team committed to delivering and improving a high standard of nursing care within Coventry and Warwickshire Partnership Trust.


Take a lead role in navigating referrals from telehealth care and healthcare professionals to the appropriate service, working autonomously and prioritising workload according to the demands of the service.


Work collaboratively with multidisciplinary teams and agencies, which may include health and social care services to support navigation of patient referrals into services which best meet their needs.

Will support patient flow out of hospital and through relevant Community Services

To assist managers and clinicians with service development and project work, data capture, audit, and reporting.

Will assist in facilitation of virtual MDT reviews including Docobo telehealth virtual care home ward rounds.
