Where our vacancy is:
We have a vacancy at our ALD Service in Ellesmere Port/Neston.
Working as part of a team to provide personalised care and support by following individual care plans and risk assessments so that service users are safeguarded.
- Caring for and supporting service users by following care plans and risk assessments so their physical, emotional and mental health needs are met, and they meet their full potential and independence
- Keep full, accurate and uptodate records including risk assessments, care and support plans for service users and daily record of events while on duty.
- Support service users to manage their own emotions and behaviours and where possible to encourage and help them to do things for themselves e.g. dressing, washing, eating, socialising and managing their finances.
- Administer or support with medications.
- Assist service users with advice and support to help them manage their finances a well as providing support around the maintenance of their homes.
- Act as Key Worker using a personcentred approach complete monthly reviews of risk assessments, care and support plans and contribute to six monthly reviews.
- In line with care plans assist with risk assessments for events/access to the community for service users individually or in groups e.g., leisure facilities, libraries. Support service users with activities taking part where necessary e.g. shopping, days out, swimming.
- Build links within the community so service users can access facilities and services e.g. social, leisure, employment and education.
- Safeguard service users at all times taking necessary actions immediately and reporting any concerns or issues to management.
- Work positively with managers, colleagues and others e.g. social workers and health professionals to fully support service users. And encouraging feedback so we improve the care and support we give.

- With dignity and care to maximise their independence, putting them at the centre of everything you do
- Dressing, feeding, personal care (bathing, toileting and continence care), with finances and giving medications
- Accessing the community, shopping, clubs, groups, swimming, socialising with family and friends joining in too
- Cleaning, cooking, ironing
- Keep accurate and uptodate records and contribute to and follow care plans
- Ensure they are safeguarded at all times
- And work flexibly as part of a team on early, late and weekends shifts
We are looking for someone who
- Shares our values
- Has a commitment to people with learning and other disabilities
- Has the ability to learn and develop skills and a commitment to gaining level 2 QCF/NVQ in Social Care (or who already has it)
- Willing to undertake personal care and able to undertake manual handing tasks
- Able to support people in the community e.g. shopping, swimming, clubs and activities
- Reliable, flexible and a team player
