PCN Care Coordinator - Administrator

South and East Leeds GP Group

JOB SUMMARY
The role is a support role for the Clinical Team. To help co-ordinate patient care by carrying out administrative and communication tasks as required to free up clinical time.

The work will involve contacting patients on the phone and answering incoming calls, processing documents, and adding codes, arranging and amending patient appointments, amending or adding clinical rotas and information to computer systems as directed.

To help senior management where practical, organising events, training, general administration and taking notes at meetings.

Responsibilities may include but not be limited to the following:

To communicate with patients over the phone in a professional manner on behalf of the PCNs creating a positive patient experience.


  • To accurately carry out all duties, effectively and efficiently in line with guidance and protocols.
  • To accurately record details on the medical records system.
  • To take full responsibility for areas of work assigned and to help and support others to complete all required tasks within the PCN.
  • To take responsibility for any patient enquiry and follow through to conclusion.
  • To deal with patient information in a confidential manner
  • To book appointments as required, accurately, ensuring patients are fully aware of full details and expectations
  • To deal with administration requests from PCN colleagues and other members of the PCN team as appropriate.
  • To have a thorough knowledge of all PCN Services, policies & procedures.
  • To be vigilant in reporting anything of concern re safeguarding children or vulnerable adults.
  • To ensure up to date with all Mandatory training.
  • To be prepared to work flexibly to help meet the needs of the PCN.
  • To carry out any other duties which may from time to time be required for the efficient running of the PCN.
  • Any other reasonable duty that may be required to ensure the smooth running of the PCN and member practices.

  • Ensure compliance with policies, procedures and guidelines for self and others, by taking action or alerting management if the PCN or member practice appears to contravene policy, or if there are concerns over any aspect of patient care.
