Description

We are looking for a full time permanent Patient Pathway Co-Ordinator to work within the Surgical Divisional POD's (Point of Delivery).

You will facilitate the smooth running of the patient journey by ensuring that co-ordinated and streamlined administrative processes revolve around the patient and their individual needs.


You will provide administrative support to clinical staff within the Surgical POD'S including Consultants, Nurse Specialists and Junior Doctors ensuring effective co-ordination of their diaries etc.

This will include cancellation and re-instating of clinics in line with planned and unplanned leave.

The team consists of 4 patient pathway co-ordinators and 1 patient pathway assistant and supports 11 surgical consultant specialists. The Colorectal POD works along the Breast POD and occasional cross-over might be required.


Co-ordinate, monitor and initiate appropriate changes within administrative processes along the patient pathway to ensure the patient's journey is managed efficiently, smoothly and in accordance with targets and agreed timescales.


Support the clinical team, Service Unit Managers and Patient Access Team in ensuring the specialities are able to deliver safe and effective services whilst also ensuring compliance with contractual access targets e.g.

the 18 weeks Referral to Treatment target (RTT).


Provide cross cover for other Patient Pathway Co-ordinators to ensure a consistent and effective administration service is maintained at all times.

Utilise computerised tracking systems to locate case notes and investigation results


Computer systems include:

  • Big hand digital system
  • Electronic Medical Records
  • Electronic Patient Records
  • PAC
Liaise with teams outside of the speciality e.g GPs where appropriate to ensure seamless patient journey.

Ensure patient information is maintained and updated on the appropriate patient information system.

Monitor work lists for Choose and Book to monitor workflow and escalate to Operational managers if necessary.

Monitor patient attendance to appointments and re-arrange/book further appointments as required.

Book patient's TCI'S and pre-assessment appointments.

  • Providing administrative support to patients, doctors and management by coordinating patients' journey through their pathway.
  • Inputting of data onto Trust systems e.g. PAS and ensuring that data remains up to date and in good order.
  • To ensure that efficient office and filing systems are maintained (both manual and electronic) so that information is kept up to date, appropriately indexed and accessible.
