Come and join our small but expanding Medicines at Home team. What do we do?

The medicines at home team covers two services provided by ESNEFT, that are managed by the Ipswich Hospital Pharmacy.


We are a medicines delivery service which can be described as a service that delivers ongoing medicine supplies and, where necessary, associated care, initiated by the hospital prescriber, direct to the patient's home with their consent.

Our purpose is to improve patient care and choice of their clinical treatment. The team is currently managing over 4000 patients.

OPAT (Outpatient Parenteral Therapy) - We facilitate patients to have Intravenous (IV) antibiotics safely at home.

We currently use two OPAT pathways to support patients in the community with IV antibiotics, with the community nursing team attending patients daily or through self-administration (SAM) where we train patients or a nominated relative/carer to safely administer their own antibiotics.

This supports admission avoidance and reducing patient's length of stay.


You will maintain maintain patient electronic records and paper files and liaise with Prescribers, Clinics and Patients regarding concerns, queries and complaints.


You will also be responsible for attending Homecare meetings with external providers and regionally as necessary to review Key Performance Indicators.
