The Rehabilitation Health & Care Support Worker delivers delegated care/tasks across a range of disciplines as part of a team-based approach to delivering holistic patient centred care.


A key aim of the role will be to support the patient to achieve optimum independence by assisting in the provision and monitoring of rehabilitation services; implementing an agreed individualised plan of care as determined by the team.


The post holder is expected to always treat individuals with dignity and respect recognising their wishes and beliefs, empowering the patient to have choice and control of their care.


The post-holder will practice in accordance with Health Board standards and statutory requirements and operate within the boundaries of the role and assessed competencies.


The post-holder will be expected to develop within the role as required within the boundaries of the job description and agreed competencies.


The post holder must communicate effectively at all times and in conjunction with other team members contributing to and maintaining the team/ward philosophy.


The post holder will assist the Nursing Team in the delivery of a rehabilitation service to In-Patients in Bronglais General Hospital, Aberystwyth, on a full time, permanent basis.

This will involve a range of therapy interventions and treatments to help patients reach their optimum level of independence for discharge.

The post holder should have previous experience working within a health or social care setting.

You will be expected to work alongside both the therapy and nursing teams on our Acute Stroke/ Frailty Ward to help promote a productive rehab pathway from assessment to discharge.


You will need to be able to work well as part of a multidisciplinary team, be self-motivated, able to prioritise delegated tasks, and show a willingness to undertake self-development.


You must be well organised, be a good communicator, and be willing to work flexibly in new and changing environments.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

  • 48 general practices (four of which are managed practices), 47 dental practices (including three orthodontic), 99 community pharmacies, 44 general ophthalmic practices (43 providing Eye Health Examination Wales and 34 low vision services) and 17 domiciliary only providers and 11 health centres;
  • Numerous locations providing mental health and learning disabilities services;
  • Highly specialised and tertiary services commissioned by the Welsh Health Specialised Services Committee, a joint committee representing seven health boards across Wales.
