Description
An exciting opportunity has arisen for Qualified Social Workers to provide Discharge Facilitation Services within the Integrated Discharge team.

The pay rates would be up to £32 per hour depending on your location and this may be negotiable.


Responsibilities of the role:


  • To work with service users and their carers, to promote and maintain their dignity, choice and independence. To assist as part of the multi disciplinary team in improving quality of life by, devising reenablement programmes to maximise independence andassist with accessing resources to provide practical help, where appropriate.
  • To give advice and information, and signpost people to appropriate provision in the voluntary and independent sectors.
  • To facilitate the safe discharge of service users from hospital.
  • To assess the needs of older people and people with physical and learning disabilities or mental health problems, and their carers.

Reed are now offering a joining bonus of up to£250 for Qualified Social Workers. Terms apply.

Requirements of the role:


  • Social Work England Registered
  • Minimum 2 years of experience in Hospital Discharge Team and similar services
  • Able to drive and hold UK Driving License
