At Thrive Tribe we deliver award-winning healthy lifestyle and wellbeing programmes that make people happier and healthier. It's a really exciting time to join us as we begin to deliver the Heathier You NHS Diabetes Prevention Programme (NDPP) throughout Cornwall, and we're looking for a group of passionate practitioners to join us on a casual contract basis as part of our bank team


The purpose a practitioner is to improve lives, through empowering people to make effective long-term changes that halt or reverse the progression of Type 2 Diabetes.

This is achieved through successfully delivering our quality programmes and providing evidence-based information, supporting individuals to improve their health, lose weight and ultimately lower their risk of developing diabetes.

We are seeking to provide county-wide coverage of this programme throughout Cornwall, therefore we're looking for flexible practitioners who can deliver the service in Penzance and surrounding areas when referrals are high.

A typical day as a
Diabetes Prevention Practitioner might include:

  • Delivering evidence-based interventions that focus on diet, nutrition, physical activity and behaviour change.
  • Facilitating group intervention sessions to support your clients in achieving their goals and cultivating a safe and positive environment.
  • Promoting the Healthier You NHS Diabetes Prevention Programme internally and externally, and improving referral pathways between other services and partner organisations.
  • Engaging in ongoing training relevant to your role, such as Behaviour Change Level 1 and
  • Working towards criteria and targets established within the National Diabetes Prevention Programmes (NDPP) quality assurance framework, including all Clinical Governance procedures.
  • Working with colleagues across all of our Thrive Tribe services to contribute to new projects and service developments we're always looking to share knowledge and resources
  • Ensuring due consideration and adherence to our internal Thrive Tribe policies, including Information Governance, Safeguarding and Complaints procedures.
  • Helping us maintain the ethos of Thrive Tribe by driving our organisational culture forward across our services and using every opportunity to embed our values

Contract:
Zero hours contract, Permanent


Our Diabetes Prevention Programme consists of 13 sessions over a 9 month period, ordinarily we expect that you will be delivering multiple sessions per month.


Salary:
£18 per hour
