Description

We are seeking an individual with previous experience to join our Angus Community Rehabilitation Team within the Angus Health and Social Care Partnership, based in Arbroath Infirmary.

The post holder will work across services including Community Hospital In-Patients, Domiciliary, Enablement Response Team (ERT) and Enhanced Community Support (ECS).

Our AHP support workers have a key role in the transition from hospital to home for patients and their families.

This post will involve both Occupational Therapy and Physiotherapy duties. You will work closely with both Occupational Therapy and Physiotherapy teams and within the multi-disciplinary team.

You will have the ability to liaise effectively with the multi-disciplinary and multi-agency team within the locality and will be able to work independently.

You will also be required to assist in out-patient based rehabilitation groups.

It is desirable that you have worked previously as an AHP support worker and previous experience working with older people in a clinical setting would be advantageous.

Hours of work are: 37.5 hours. (part-time hours considered)

You will be required to comply with the Code of Conduct for Healthcare Support Workers in Scotland throughout your employment.
