Salary - £450ltd per day

Description:

Work with colleagues across the People Strategy and Commissioning team to deliver the Adult Social Care Transformation Programme Lead on complex commissioning and transformation projects within Adult Social Care, Mental Health, Physical and Learning Disabilities
Implement commissioning activities end to end in line with processes agreed with the Integration Manager against strategic health and social care outcomes
Oversee activities to enable individuals to live as independently as possible and delay the need for highcost care through commissioning high quality communitybased services
Undertake monitoring and evaluation of commissioned services to identify gaps to deliver innovative and creative solutions for sustainable change.

What is required?

Educated to degree level or have equivalent experience
Previous experience within adults commissioning
