Description
The postholder will work as an integral part of a multi-professional team working with individuals with severe and enduring mental health problems who have problems in engaging and maintaining lasting and meaningful contact with mental health services.
The nature of the client group and the philosophy of the team will require the post holder to work flexible hours when required, core working hours will be 9-5 Monday to Friday.
The post holder will be responsible for the assessment of care needs, the development, implementation and evaluation of recovery based care programmes and the setting of standards of care for this complex group.
To have the responsibility for the delivery of an Assertive Outreach/Intensive Support approach/intervention to specific individuals.
Carry out all relevant forms of care without direct supervision.
Coordinating clinical activity, liaison with the rest of the Community Mental Health Team (CMHT) and other appropriate teams, services and agencies.
Immediate problem solving and decision making and contributing to long term planning for future developments both within the immediate clinical area and within the service area.
To ensure that the values and principles of the organisation and of the service area are translated into the philosophy and operational policy of the clinical team and that these are evident in everyday practice.

Contribute towards the management of clinical activity within the CMHT in relation to the specific service user group, including issues relating to referrals, assessments, activity levels, caseload numbers and clinical risk.
Ensuring that local policies and procedures are adhered to in respect of the above and that where a clinical situation is not provided for by policy / procedure that the communication and decision making process is clear and well documented.
Positively promote the service and good practice therein both within the organisation and with outside agencies. Seek out opportunities to develop networks beyond the immediate team to enable sharing of good practice and benchmarking.Monitor and maintain standards in relation to administrative tasks ensuring timeliness and accuracy in respect of the same. (E.g. notes, expense claims, time sheets ordering and administration of medicines.)
