Working autonomously as an Advanced Practitioner, the post holder will have responsibility for proactively monitoring & managing patients, recognising the early symptoms of disease exacerbation, acute illness & injuries based on the understanding of long-term conditions, disease processes, current evidence and practice standards.

The service is delivered to a population of patients within the community setting primarily residing in care homes. The key aim of the service is prevention of inappropriate hospital admission or readmission following discharge from acute care. In achieving these outcomes there will be a reliance on comprehensive physical assessment and treatment-planning along-side EHCPs and DNACPR.

Post involves liaising with health/social care providers & other agencies to ensure provision of high quality care management to this group of patients/carers.

Post-holders will also forge strong links with the nurse-led unit and community hub which is located on the University Hospital of Hartlepool site delivering pathway based interventions.


The Community Matron service operates over 7 days a week, covering the hours of hrs across the Hartlepool and Stockton localities.


The Community Matron service is a team of highly skilled advanced nurse practitioners working within Care Group 1 of the Trust (Healthy Lives).


The matron deliver reactive & proactive interventions to people residing in care homes across the Hartlepool & Stockton CCG area.


The key focus of this team is to ensure that people are cared for in their own home environments & that hospital admission is prevented where possible.
