Milton Keynes.

From 1st of September 2023 we will be delivering excellent care services to patients and communities across 3 sites in Milton Keynes

Brooklands
,
Whitehouse and
Neath
Hill surgeries.

We have an amazing job opportunity for a
Community Link Worker to join our team.

As members of the PCN's team of health professionals, the community link worker will take referrals from the PCN's Core Network Practices to support the health and wellbeing of patients

Working with and reporting to the Senior Operations Manager for Primary Care, they will assess how far a patient's health and wellbeing needs can be met by services and other opportunities available in the community

They will co-produce a simple personalised care and support plan to address the patient's health and wellbeing needs by introducing or reconnecting people to community groups and statutory services, and signposting where appropriate and as it matters to the person


Knowledge, training and experience required:


  • The community link worker must have completed the NHS England and online learning programme accessed via the Personalised Care Institute website
  • Must be enrolled in, undertaking, or qualified from appropriate training as outlined by the Personalised Care Institute Core Curriculum and social prescribing link worker competency framework
  • Must attend the peer support networks delivered at place or system by the ICS in the region
