Description

To make onward referrals and signpost to other services including VCSE and neighbourhood services to enable wrap around, holistic support for service users undergoing treatment to meet a broad range of need To collaborate with the service user to enable the individual to be a partner in care To participate in the induction, training and education of students and other staff from all disciplines.

To participate in any investigations into serious incidents or learning reviews and contribute to action plans to prevent future occurrences To respond appropriately to concerns or complaints raised regarding theservice or own practice according to Trust procedures and guidelines and inconjunction with team manager, using outcomes to improve service delivery To strive to provide an excellent standard of care and positive serviceexperience, working with experts by lived experience to shape and developthe service.

To participate in data collection, audit and research as required To ensure all records are entered and maintained according to TrustStandards for Record Keeping and with due regard to the IG governancestandards and procedures.
