Description

An exciting opportunity has arisen within the Brokerage Service for two Brokers who will focus on community and hospital discharge referrals, primarily sourcing home care support for people, but also supporting as required in sourcing other services such as care homes or supported living.

The roles will involve liaising and negotiating with care providers and families, as well as working closely with Health/Adult Social Care colleagues.

The vacancies are on a temporary fixed term contract until 31 March 2025 due to funding, working on a Monday to Friday basis.

It is possible that the posts may be extended however this depends on the requirements nearer the time and funding availability.

Whilst the post will be initially based at Clay Cross and/or Chesterfield during induction, we offer a hybrid approach to working, with our team currently working from home and from the office at least one day per week, to meet the needs of the service.
