OVERVIEW:


Community Voluntary Services Tendring (CVST) has been working in the local community for many years and provides a range of resources and support to vulnerable people across the Tendring district.

CVST plays a key role, acting as a central hub in getting the help to vulnerable, isolated and homeless people and working with partners across the district, to support people through Social Prescribing to reduce loneliness, increase confidence, encourage people to volunteer and strengthen local communities.


PURPOSE OF THE POST:

The Community Connector will be part of CVST's Healthier Independent Longer Lives (HILL) Team.

Working closely with the Social Prescribing Team, individuals, the NHS and the voluntary and community sector in Tendring, the Connector will support people in their local community to engage in activities, gain new skills, increase physical activity and volunteer.


Main Duties

  • To connect with individuals in the community at CVST's premises and at Kennedy Way Community Hub by attending clubs, groups, activities and events. To support and engage with those individuals to take control of their wellbeing, live independently and improve their health outcomes.
  • To take referrals via the Priority platform, from partner organisations and selfreferrals from individuals and to complete, process and input referral forms and surveys.
  • Work with individuals to coproduce a simple personalised support plan to improve their health and wellbeing. This may include introducing them or reconnecting them to activities, community groups, volunteering and local services including, in particular, at Kennedy Way Community Hub, Men's Shed and other groups within the Kennedy Way Community Garden project.
  • Support individuals with accessing transport to attend clubs and activities.
  • Support individuals to access local training opportunities.
  • Develop trusting relationships giving people time to focus on 'what matters to them' and follow up with them at 3, 6 or 12 month intervals as appropriate.
  • To maintain accurate records
  • Work with CVST colleagues to help out at events including summer safety, Town Centre Community Fair, winter resilience activities, cultural awareness activities, training and other adhoc activities as needed throughout the year.
  • To support and facilitate the development of new and existing activities at Kennedy Way Community Garden, Hub and Men's Shed.
  • To coordinate staff and volunteer rota at Kennedy Way to ensure that an efficient and consistent presence is maintained on site throughout the week.
  • Work in partnership with the CVST Facilities Team to support hire and activities at the Kennedy Way Community Project.
  • To work with the HILL Team to provide wraparound support for individuals through Social Prescribing.
  • Working In partnership with the CVST Volunteer Coordinator and the Social Prescribing Team, support and utilise volunteers.
  • To work closely with the Kennedy Way Medical Centre and CVST Social Prescribing Link Worker within the surgery to support and engage individuals to take control of their wellbeing, live independently and improve their health outcomes.
  • To provide reception cover for the Community Information Centre and other CVST projects as required.

General (all CVST staff)

  • Keep up to date with relevant legislation, policies and practices, including undertaking research and attending appropriate training courses as required.
  • To meet with and report to the Communities Manager on a regular basis.
  • To adhere to and promote secure procedures regarding personal data and data protection.
  • Promote equal opportunities, equality, diversity and inclusion in line with the organisation's Equality and Diversity Policy.
  • To adhere to all of CVST's operational policies and procedures including but not limited to Equality & Diversity, Health & Safety, Safeguarding and Financial Systems.
  • Work with the team to ensure that risk assessments are in place for events and activities relating to the programme.
  • To comply with individual responsibilities, in accordance with the work role, for health and safety in the workplace.
  • Attend and participate in staff and team meetings and other CVST meetings as appropriate.
  • To work from various locations across the district including CVST's Harwich Centre at 276 High Street, Dovercourt on occasion.
  • Attend all mandatory training (online or inhouse) as necessary
  • Positively represent and promote CVST to external agencies
  • Undertake any other duties and tasks appropriate to the post as may be required from time to time.

Part-time hours: 22.5 per week

£21,000.00 per year
