Sanctuary Personnel is currently seeking an experienced Band 6 Complex Discharge Facilitator to take on a new locum opportunity based in Worcester.

Contract Type:
Locum, 3-month ongoing contract


Pay Rate:
£26.48 per hour (assignment rate)


Shift Pattern: 37.5 hours per week


Duties:


  • Support the ward staff with complex discharges taking the lead in discharge arrangements for individual patients where appropriate, liaising effectively with all members of the Multi-Disciplinary Teams in Community settings
  • Act as a resource for patients, relatives and carers requiring information on discharge processes, services available and all other issues related to discharge
  • Regularly attend MDT meetings within our Inpatient Wards and Rehabilitation Units to work alongside the clinical teams in ensuring that patients are supported to be discharged in a timely manner
  • Lead on the coordination of 117 aftercare plans and support with wider teams and the local authority
  • Manage your own caseloads of patients and prioritise cases where necessary
  • Use your initiative and take responsibility for yourselves and the quality of your work and the service you provide to patients
  • Assist in finding appropriate placements, supported living and residential care for patients who require this level of support upon discharge

Requirements of the Complex Discharge Facilitator**:

  • RMN/Social Worker
  • Diploma / Degree or working towards
  • Evidence of portfolio of knowledge, skills and professional development
  • Ideally have prior experience working in this environment
  • Significant relevant post qualification experience
  • Experience of liaison with other agencies, e.g. social care, voluntary sector
  • Experience of the funding processes for NHS funded care and social care
  • Discharge planning experience
  • Experience of MDT working
  • Experience of conducting and contributing to ongoing assessment
  • Experience of the CHC process (desirable)
  • Specialist knowledge of a range of practices and procedures
  • Knowledge of health and social care services in the community
  • Knowledge of current NHS developments
  • Knowledge of National and local health and social care strategies (desirable)
  • Knowledge of patient risk assessments for discharge (desirable)
  • Ability to challenge poor behaviour
  • Skills to role model good behaviour
  • A positive attitude and able to act on feedback
  • Wide variety of evidence based clinical skills
  • Assertive
  • Enthusiastic and forward thinking
  • Proactive and selfmotivated and ability to work as part of an MDT
  • Ability to cope and remain calm under pressure
  • Able to meet the travel requirements for the post
