The community team of West Essex is part of the wider Hertfordshire and West Essex Integrated Care System, supporting out of hospital care for West Essex residents.


To provide leadership to a multidisciplinary Integrated community care team to deliver patient centred care to people with a focus on rehabilitation.

This will involve helping patients to engage as independently as possible in activities which enhance their health & wellbeing. Interventions will be carried out within patients own homes, 24 hour care facilities and clinics.

To be responsible for ensuring the delivery of high-quality person-centred services. day to day management and leadership of therapy and assistant practitioners.

To provide senior clinical advice, leadership and support ensuring the needs of the service are met by exercising and demonstrating high levels of clinical judgement, critical risk assessment and advanced decision making skills.

Flexible working is required, as at times you will be expected to work across sites in our other West Essex localities depending on service needs.

To act as a senior decision maker to guide and support staff with complex patient cases.


To assess, plan, implement and evaluate specialist treatment and care and/or rehabilitation programs to people, promoting independence and autonomy; working within a multidisciplinary team.

Provide expert clinical holistic assessment and evidence based pro-active and reactive health care interventions to support patients and their carers to achieve maximum health welling and independence

To participate in duty clinician cover, triage and MDT/allocation meetings where clinically appropriate.

To act at all times as an advocate for service users, carers and relatives.


To be responsible for standards of care within the service area; to evaluate clinical care and develop actions with relevant line managers where required.

To have overall responsibility for caseload management through continual monitoring and data collection.


To maintain a professional portfolio and demonstrate that practice is up to date and evidence based through regular management and clinical supervision/mentorship.


Day to day management and leadership of therapy and at time nursing practitioners and other staff in the multi-disciplinary team.

Here at EPUT we maintain our own bank of specialist staff to ensure that we are able offer safe and effective care at all times.

Communication Skills:


Using verbal, non-verbal and written communication tools ensure that patients and carers have an understanding of their condition and the role and aims of Therapy in relation to this.


To be confident in their own diagnosis and be able to communicate that effectively to others (including medical doctors, other Allied Health Professionals etc).


To share information in a timely manner with more senior staff if there are any concerns about a patients safety, condition or prognosis.

To urgently report and take, appropriate action if there are any concerns about staff and/or patient safety.


To represent the community service in meetings with other professionals with an aim of accurately contributing and reporting back relevant information to their line manager.

Maintain confidentiality at all times, consistent with Trust policy.

Maintain accurate clinical records in accordance with professional and trust standards.

Use IT skills accurately in patient notes and report writing.


Professional Skills:


The staff member must be able to adapt their communication, treatment, information and advice to meet the needs of individual patients.

Patients seen will range from the vulnerable to those who are extremely well informed about their condition.
