This role will involve duties such as managing referrals received via the Unified Point of Access, providing clinical support to our large multi-disciplinary team and other administrative tasks such as managing an inbox and taking patient or clinical queries over the phone.

This role will involve shifts between 08:00 and 20:00 and weekend working. One weekend to be worked in four weeks (once a month) on average.

The post is based at 2 sites, Elmcourt, Tulse Hill, SE27 & Walworth Road Clinic, Walworth, SE17.

The service operates 7 days per week, from 08.00hrs to 23.00hrs. Patients using the service will range from those requiring a single episode of care e.g.

postoperative to IV therapy for rehydration, antibiotic therapy; through to patients living with complex and long term conditions, including the frail elderly, who may require periodic short term intervention to maintain the best possible quality of life at home.

The service accepts referrals from GP's, London Ambulance Service (LAS), Guys and St Thomas Hospitals, Kings College Hospital, St George's hospitals as well as other community services.


  • Same day or next day response to support early dischargefrom hospitals from Guy's & St Thomas, King's College and is integrated with secondary care.
  • Hospital admission avoidance, with 2 hour or same day response, working in conjunction with London Ambulance Service (LAS), GPs, Community Matrons, District Nurses, Social Care and the other community therapy teams.
  • Excellent organisational and prioritisation skills with the ability to maintain attention to detail and multi task in a busy environment.
  • As they will work closely within a multidisciplinary team to provide high standard administrative support, they will therefore also need to possess strong interpersonal and communication skills, both written and verbal.
  • They will be able to work well autonomously but also as part of a team.
  • They will have good knowledge of IT systems and competent in Microsoft Office packages.
